FIRST OF ALL, IN MY OPINION, UNIFORMITY FORMAT SHOULD BE ADDRESSED, IDENTICAL FORMATS ACROSS THE BOARD WHETHER IT BE INTERNET OR PAPER FORM FOR ALL ENTITIES. MY OWN IDENTITY IS SO COMPROMISED THAT I DO NOT KNOW WHEN IT BEGAN. SOME FORMS ASK FOR FIRST LAST NAME, OTHER FIRST MIDDLE INITIAL LAST NAME, OR FULL BIRTH NAME. THERE ALONE A DATABASE ENTRY MAY, WILL IDENTIFY ME AS THREE DIFFERENT INDIVIDUALS, SAME BIRTH DATE, SAME PARENTS. WHERE DOES IT BEGIN, THE DOCTORS' PERSONNEL, THE PHARMACY, FINANCIAL EMPLOYEES, REAL ESTATE INDUSTRY, INSURANCE INDUSTRY OR ONE'S OWN FAMILY MEMBERS. THE SOCIAL SECURITY NUMBERS SHOULD HAVE NEVER BEEN USED AS AN IDENTIFIER WHERE OVER THE YEARS ID THEFT HAS ESCALATED TO OVERWHELMING PROPORTION. WITH COMPANIES OUT-SOURCING FOR COST EFFECTIVENESS ONE'S IDENTITY IS ALREADY COMPROMISED UNLESS YOU ARE A GOVERNMENT EMPLOYEE (ANY BRANCH), OR MILITARY. IT BEGINS WITH THE SAME ENTITIES AND INDIVIDUALS THAT YOU MUST GIVE ACESS TO IN ORDER TO RECEIVE ANY TIME OF SERVICE AS PREVIOUSLY MENTIONED. FOR EXAMPLE THIS FORM FIRST AND LAST BUT I HAVE A MIDDLE NAME/INITIAL! THE SSA NEVER INFORMED ME THAT MY SSN/IDENTITY HAS BEEN/IS COMPROMISED AND YET IT IS. HOW EASY IS IT FOR A SSN BE TRANSPOSED IN A DATABASE AND NO ONE PICK UP THE ERROR UNTIL TOO LATE AND AT WHAT COST. WHILE IMPROVEMENTS HAVE BEEN IMPLEMENTED WITH THE SSN ON BIRTH/DEATH CERTIFICATES OTHERS SUCH AS MYSELF ARE VICTIMS. A SYSTEM SHOULD BE PLACED THAT WOULD PROBABLY OVERSEE THE DATA ENTRY OPERATOR INPUT AND BLOCK/LOCK ERRORS OR MATCH TRANSPOSED SSN WITH SIMILAR AND ACTUAL AND SOMEONE WILL FIND A PATTERN. IN MY CASE, THERE HAVE BEEN DOUBLE STATE FILING, ONE WITH FIRST LAST NAME, THE OTHER WITH FIRST, MIDDLE INITIAL, LAST NAME, AND I CAN NOT EVEN GET THE FEDERAL TAX DOCUMENTS OF THE EARLY '80 THAT WOULD HELP ME SOLVE THIS.
