This is redundant requirement and should not be passed. We provide everyone of our patients a copy of their contact lens prescription with an appropriate expiration date as required by law (as with their spectacle correction). To require us to have the patient sign a form that we have provided it and keep it on file for 3 years is another unnecessary administrative task added onto the already over burdened doctor who has to deal with electronic health records, meaningful use reporting, MIPS and PQRS reporting. We are already drowning in regulation and paperwork please do not put another redundant requirement around our necks.
