This additional paperwork requirement does not help consumers. It adds cost to small businesses and aggravation to patients in having another form to sign. But the worst part is that it drives a wedge of mistrust between patients and their doctors. The form begs the question, "Why does the government not trust my doctor to follow the law?" Optometry and ophthalmology practices are built up by trust. This rule harms that trust for the thousands of doctors who are already compying with the law.
