I do not want to receive automated phone calls, unless I specifically authorize a merchant to call me about a sale or the like. These automated calls are worse than the old telemarket calls because you cannot tell a machine to stop calling or to go to hell. P.S. On this form you use the term "postal code". This is the United States of America, you are an agency of the US government. Please use American terminology, you know, Zip Code?
