Dear Sir: This is America. The land of free opportunity. If you require this, you should require me to fill out a form at the gas station, resturants..What are you thinking? This is a violation of our constitutional rights. Americans do not need your "protection". We are not stupid. By the way. I am a minister. You do not even acknowledge me with your form but yet you want to control how and who I might choose to do business with.
